Nattokinase carries some of the boldest claims in the supplement aisle, and some of the most serious safety caveats. Both deserve to be stated plainly.

Every human trial behind the blood-pressure claim used roughly 2,000 FU/day of a standardized, enteric-coated extract — the coating is meant to get the enzyme past stomach acid — for 8 weeks in the pivotal RCT, with durations varying across the 6 pooled RCTs. Nothing in the evidence supports going higher: lipid effects were inconsistent and dose-dependent, bleeding is the dose-limiting risk, and there is no established dose at all for "dissolving microclots" or Long-COVID because no human dosing trial has ever tested that outcome.

Ceiling: Evidence stops at about 2,000 FU/day over 8 weeks. No studied dose above that, and no dosing trial of any size for microclot or Long-COVID use.

The evidence

The viral claim that nattokinase dissolves clots or microclots and reverses Long-COVID has never been tested in a single human trial — it rests entirely on test-tube assays where the enzyme is dripped onto fibrin outside the body.

Safety and interactions

Common side effects

  • Generally well tolerated; supplement use up to ~3 years appears safe in available reports
  • Occasional, mild gastrointestinal upset (anecdotal, not a theme in the pivotal trials)
  • Bleeding or bruising — rare but serious, and the primary safety concern
  • Allergic reaction — uncommon; it is a soy-fermentation product
  • Variable vitamin K2 content depending on the specific product

Interactions that matter

  • Anticoagulants (warfarin, apixaban, rivaroxaban, dabigatran) — additive interference with clotting; avoid without direct medical supervision
  • Antiplatelets (aspirin, clopidogrel) — additive antiplatelet/fibrinolytic effect, increased bleeding risk
  • Other fibrinolytic or thrombolytic drugs — directly additive mechanism; avoid the combination

Questions people actually ask

Will this dissolve clots or microclots in my body?

No human trial has ever tested that. The claim comes from lab assays where the enzyme is added directly to blood plasma or fibrin plates outside the body. That cannot show a swallowed capsule survives digestion, is absorbed intact, reaches a clot, and does the same thing inside a living person. Graded Insufficient.

Does nattokinase help with Long-COVID?

There is no published randomized controlled trial testing nattokinase for Long-COVID symptoms or microclot burden. The narrative traces back to in-vitro research and to content published by sellers of specific purified nattokinase extracts. Graded Insufficient. There is also no established dose for this use, because no human dosing trial has tested it.

So what does it actually do?

It lowers blood pressure modestly. A meta-analysis of 6 RCTs in 546 people found systolic down 3.45 mmHg and diastolic down 2.32 mmHg. An independent 8-week RCT in 86 pre- and stage-1 hypertensive adults found 5.55 and 2.84 mmHg at 2,000 FU/day. Real and statistically significant, but roughly the range of cutting salt.

Can I take it with aspirin or warfarin?

Not without medical supervision. Nattokinase's fibrinolytic action is additive with warfarin, DOACs like apixaban and rivaroxaban, and antiplatelets like aspirin and clopidogrel, and case reports of bleeding events exist. Stop before any scheduled surgery or dental work. On warfarin, also check the product's vitamin K2 content, which pulls the opposite way.

Is nattokinase FDA-approved?

No. It is sold in the United States as a dietary supplement, not an approved drug, and carries no FDA-approved claim to treat, prevent, or dissolve blood clots. Natto itself is a traditional Japanese food with centuries of dietary use, but the concentrated enzyme has never gone through drug approval for any thrombosis-related indication.

What about cholesterol and triglycerides?

Pooled trial data show inconsistent, dose-dependent effects on lipids, alongside a small rise in blood glucose in some trials. We grade this Weak — not a clear or reliable benefit. If improving your lipid panel is the reason you are considering nattokinase, the current human evidence does not support that purchase.
